Are failed initial tests (not retests) recorded as violations? Is there a charge associated? Are temporary lockouts tracked as well?

Yes, failed tests are recorded as violations. No, we do not charge for failed tests. Yes, lockouts are tracked as well as many other data points. Hope this helps!
